The Only Cure+ · Field Surgery expands limb amputation, recovery and prosthetic equipment for Project Zomboid Build 42. This standalone fork of The Only Cure includes the complete mod and requires no other mods.
Enable either The Only Cure or The Only Cure+, never both. “Field Surgery” is part of the name, not a separate dependency.
Surgery with consequences
Examine a wound to assess infection spread. Amputating within the configured cure window can remove the infection; waiting too long turns the procedure into a gamble.
Surgery proceeds through disinfection, cutting, sawing, stitching and bandaging. Each stage requires suitable tools and carries its own risks. A checklist shows the odds before you begin.
Recovery includes shock, convalescence, phantom pain and possible sepsis if the stump is neglected. Medical professions know the procedure; other survivors must read the Field Surgery Guide.
Modular prosthetic arms
Combine five bases with seven attachments:
- Bases: wooden, scrap, mechanical frame, medical and cybernetic.
- Attachments: wooden knob, hook, mechanical hand, medical hand, cybernetic hand, chainsaw and bayonet.
Compatibility restrictions apply. Chainsaws need metal bases, mechanical and medical hands cannot fit wooden bases, and cybernetic hands require a tier-three base or better.
Older arm prostheses convert into matching components while retaining condition.
Workbench, upgrades and maintenance
Build a prosthesis workbench at Carpentry 3 to fit components, compare stats and preview your character in 3D. Paint bases, attachments and legs in 10 colors, or remove the paint afterward.
Cybernetic equipment requires advanced crafting skills and rare electronics. Its frame grants +1 Strength, while the cybernetic leg runs 10% faster than a healthy leg.
Prostheses wear with use and detach at zero condition. Repair them and develop Prosthesis Familiarity to improve action speed. Wooden attachments and hooks cannot handle two-handed weapons or tasks requiring fingers; an empty base cannot hold items.
Weapon attachments and missing legs
Right-arm attachments include a petrol-powered chainsaw that attracts nearby zombies and a bayonet that still allows pistol use.
Without a prosthesis, losing one leg requires a crutch to stand. Crutch movement is tiring and cannot become a run. Without both legs and without prostheses, the character crawls and can still fight. Peg legs cannot sprint.
An optional setting also allows sufficiently strong leg hits to turn zombies into crawlers.
Compatibility and saves
Supports multiplayer with server-validated body changes. Safe to add to existing saves; restart multiplayer servers. Original The Only Cure saves remain supported.
Back up before updating from 3.2, as worn arms convert on first load.
For Tomb’s Player Body, enable the included TombBody fit add-on and load it after both mods. BB First Aid Overhaul is incompatible.
Includes 11 languages, including Russian. See the Player Guide for detailed procedures and settings.
